Why tight tolerances matter here

Three things make medical tooling demanding on the pins. First, the features are small: a core pin can be the smallest feature in the part, so its diameter and straightness land directly on the dimension that has to stay in spec. Second, the molds run hot, fast, and at high cavitation, so the same pin geometry is repeated across many cavities and many cycles. Third, medical tooling is often validated, so a replacement pin has to reproduce the original dimensions.
